This patient who had surgery initially elsewhere developed capsular contracture in her implants. The options for her were to consider exchange of the implants or consider removing the implants and proceed with reinsertion of the breast implants at a second stage. If one is to consider a two stage approach, one should leave 3 months following explanation before considering this. Often however patients are surprised at the results following simple removal.

Breast Augmentation Removal Case Study 6
This lady had significant capsular contracture with ptosis following a breast augmentation and periareolamastopexy performed elsewhere. Having discussed the options of removing only, removal and lift and the pros and cons of one and two stage implant replacement she decided to remove the implants and the caspsulesen bloc and perform a mastopexy at the same time. By doing this one can improve the cosmesis of her previous periareola scars, lift the breast into a better position and obtain an aesthetic result. She is happy with this and will not plan to have implants again.
Often patients have concerns that they will have no volume when the implants are removed. Often augmentation has been performed at a time in their lives when they have other concerns and forget about the amount of breast they had at that time. Often following pregnancy and with age one can accumulate more breast tissue. Often patients are surprised at how much breast tissue they do have following implant removal.

Gynaecomastia Case Study 1
This patient has mild gynaecomastia and has undergone bodyjet liposuction to remove excess fat from his breasts. The scars are placed in the armpit and are not visible from the front. It is important for patients to realise that not all fat can be removed through liposuction and that over time fat can return especially with weight fluctuation.

Asymmetric gynaecomastia with breast tissue on the left. Liposuction was unable to completely reduce the breast tissue significantly at the time of surgery and it was necessary to open an incision around the areola and remove some tissue directly. Occasionally the tissue can be hard and fixed and liposuction only occasionally is not sufficient to achieve a suitable result.

This lady has had a face and neck lift with midface lift. Her main concerns were the lower face and neck and midface hollowing. She required a repositioning of the SMAS and the platysma with jaw recontouring. By repositioning the internal structures of the lower face and neck by using different vectors one is able to improve all the facial aesthetics in one procedure.

This lady has had a face and neck lift utilising a classic facelift incision with SMAS elevation and lateral plastysmaplasty. The increased volume in the malar area helps to give patients back the youthful appearance.
