This lady was unhappy with the aging of her face and the way that the midface had hollowed, the jowl had dropped and that her jawline had become less well defined. A facelift that undermines the full face and neck enables one to be able to reposition the internal structures of the face in a manner that can be individualized to patients wishes. Re-volumising the face can be performed using varying vectors. Mr Ross does not discard any internal tissue within the face but repositions it to help maintain and improve volume. Excess skin often needs to be removed but the main focus of facelifting is the repositioning of the internal stuctures.

This lady has significant problems with excess fat around her thighs. For early lipoedema liposuction can provide patients with an improvement in aesthetics and quality of life. This lady has had almost 6 liters of fat removed from her legs in one operation. There is a limit to how much fat can be removed in one sitting and often with substantial fat removal there can be associated fluid shifts and drops in haemaglobin. Water assisted liposuction is Mr Ross’s preferred technique and the fluid helps to reduce the bleeding and allows more fat to be harvested. Mr Ross advises patients to have this surgery under general anaesthetic and to stay in overnight to be monitored postop. It is important that patients undergo physio with manual lymphatic drainage and Mr Ross advocates the use of the lymphotouch device to help with the recovery. The results are seen at 6 weeks and there will still me some improvement over the coming months as the oedema continues to settle. There is a limit to how much fat can be removed and patients need to be aware that as they heal that the legs can be itchy afterwards.

This lady was concerned about the excess fat around her arms. She did not wish to have an arm lift and patients must always be aware that liposuction in the arms can create excess skin that at a separate date may need to be excised. Liposuction does help with troublesome fat and in patients with good quality skin liposuction can be a good option. The results are seen after 3 months and show an improvement in the contour of the arms. It is important for patients to consult with a plastic surgeon who can offer both liposuction and arm lifting so that patients can be empowered to make a decision to proceed.

This lady has had breast implants in place for some time. She has felt that the implants have dropped and the implants have become more palpable and have expanded the skin. She was concerned about the amount of breast tissue she would have postop but the results are seen at 3 months after everything has settled. The key is to try and keep the inframammary scar in the crease so that it does not become visible.
