The areas most difficult to rejuvenate are the swelling at the lid cheek junction and the tear troughs. The malar area is very difficult to address as it is often a combination of fluid and oedema and often it is not possible to truly rejuvenate this area. Patients need to be aware of the risks of having a midfacelift at the same time as upper and lower blepharoplasty. This has been carried out in this lady and has improved the overall contour of the periorbital region.
